Can I use VB for real time image processing such as frame subtraction?
What I want to do is compare consecutive frames and see whether there is a change and put a box around it , follow it as it moves and save to file!

IC Imaging Control contains a motion detection example named "Advanced Image Processing" for Visual Basic. In this example, the user draws a rectangle on the video image. In this rectangle, the application searches for differences between the frames.
In general, all of the functions you described are possible with Visual Basic and IC Imaging Control. But in my opinion, Visual Basic could be a little bit too slow for the pixel operations you want to perform. I would prefer a C++ application.
Therefore, try the trial version, downloadable at http://www.imagingcontrol.com/ic/downloads/
and test the "Advanced Image Processing" example.
